    Understanding and implementing new Heart Failure guidelines in primary care beyond the QOF tick box

    • Apply current heart failure guidance, including the new HF009 indicator, to identify, diagnose and review patients accurately in primary care.
    • Initiate and titrate four-pillar therapy (ARNI/ACEi/ARB, beta-blocker, MRA, SGLT2 inhibitor) and run a structured review that improves outcomes rather than just meeting QOF.

    Key message: Good heart failure care means using the four pillars and proper structured reviews to genuinely help patients — not simply ticking the QOF box.

    Statins, Targets and Tolerance: Practical Lipid Optimisation in CVRM Care

    • Set and work towards appropriate LDL-C and non-HDL targets based on each patient's cardiovascular risk.
    • Recognise and manage statin intolerance, and use combination therapy to get patients to target.

    Key message: Reaching lipid targets often takes more than a statin alone, so managing intolerance and combining therapies sensibly is central to cutting CV risk.

  • Hypertension and Diabetes: Practical Combined Management in primary care

    • Manage patients with both hypertension and diabetes to appropriate BP targets, choosing medicines that benefit both conditions.
    • Apply renal protection strategies (including ACEi/ARB and SGLT2 inhibitors) and monitor kidney function effectively.

    Key message: Treating hypertension and diabetes together, with the right drug choices and renal protection, does far more to reduce cardiovascular and kidney risk than tackling each one in isolation.

    Less Common Cardiac Conditions and ECGs Care

    • Recognise cardiac red flags and interpret common ECG abnormalities seen in everyday primary care.
    • Apply appropriate referral thresholds and timescales, distinguishing what needs urgent referral from what can wait.

    Key message: Confident ECG interpretation and clear referral thresholds help clinicians spot the serious cases early and refer with confidence.

Professor Ahmet Fuat

PhD, FRCGP, FRCP, FRCPE ,FESC, FPCCS, PGDiP Cardiology

Honorary Professor, Durham University · GP & GPSI Cardiology (25+ years) · National Programme Director, CliniLink

A Darlington GP for 40 years and one of the UK’s leading voices in primary care cardiology. Professor Fuat established the UK’s first one-stop integrated heart failure clinic in 2002, earned his PhD researching heart failure diagnosis and natriuretic peptides, and has shaped NICE and ESC guidelines on CVD and heart failure — with his HF work featured in the BHF Blueprint for Heart Failure.

Fellow of the RCGP, RCP London, RCP Edinburgh, the ESC and the PCCS, he sits on the editorial boards of the British Journal of Cardiology and the Primary Care Cardiovascular Journal. He teaches because translating evidence into everyday practice is where patient care is won.

 

Dr Yassir Javaid

MA (Cantab) FRCP FRCGP PGDip Cardiology GPwSI Cardiology
Dr Javaid qualified from Cambridge University and completed his VTS training in Northampton. He has a specialist interest in cardiology and echocardiography and was a clinical lead in the Northamptonshire Community Cardiology service, which had a focus on patients with heart failure and valve disease. He was also the primary care Cardiovascular lead for the East Midlands Clinical Network from 2013-2020. He was named Pulse “GP of the Year” in 2015 for his work in reducing stroke emergency admissions in the East Midlands. He is on the editorial board for the British Journal of Cardiology and a trustee of Heart Valve Voice.
